Radiolabelled interleukin-2 (specifically [68Ga]Ga-IL2) is an investigational radiopharmaceutical designed for PET/CT imaging of activated T-lymphocytes. Developed by the Nuclear Medicine Unit at Sant'Andrea University Hospital in Rome, it consists of recombinant human interleukin-2 (IL-2), such as aldesleukin, conjugated to a chelator (e.g., NODAGA or THP) and labeled with the positron-emitting isotope Gallium-68. The tracer specifically targets the alpha subunit of the interleukin-2 receptor (IL-2Rα or CD25), which is upregulated on the surface of activated T-cells. This allows for the non-invasive assessment of immune cell infiltration and activation in various clinical contexts, including COVID-19 pneumonia and the tumor microenvironment, potentially serving as a biomarker for disease prognosis or response to immunotherapy.
